Coach Angie Ltd. (CAL) Services and Scope of Practice

1. Services: The dates and times of the Services and/or location will be mutually agreed upon by CAL and Client. Client is solely responsible for creating and implementing Client’s own physical, mental and emotional well-being, decisions, choices, actions and results arising out of or resulting from the coaching relationship and the provision of the Services. As such, the Client agrees that CAL is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any Services provided by CAL. Client understands coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease. Client acknowledges that the Services are a comprehensive process that may involve different areas of their life, including work, finances, health, relationships, education and recreation. Client agrees that deciding how to handle these issues, incorporate coaching principles into those areas and implementing choices is exclusively the Client’s responsibility.

2. Limited Scope of Services: Client acknowledges and agrees that CAL is not Client’s medical provider, and as result, CAL and its RNs will not provide any medical care, treatment or diagnosis to Client. Client further acknowledges and agrees that: (1) Client is solely responsible for arranging for all medical care and treatment on their own behalf; and (2) CAL and its RNs are not offering an opinion about any care received by Client from their medical providers. Client also acknowledges and agrees that Client has pre-existing relationships with certain medical providers and that Client will continue to receive medical care, treatment, and diagnoses from such medical providers or other providers of Clients choosing. The Services provided herein are non-clinical in nature and do not replace any medical care that Client currently receives from Client’s existing medical providers. Rather, the Services are intended to be informational and education in nature and will be solely provided within the scope of the RN’s license under thellinois Nurse Practice Act, as amended. Any questions or concerns about Client’s condition or treatment plan must be directed to the Client’s medical providers.

Client acknowledges that the Services do not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that the Services are not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental healthcare,substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. If Client is currently under the care of a mental health professional, it is recommended that the Client promptly inform the mental health care provider of the nature and extent of the Services agreed upon by the Client and CAL. For the foregoing reasons, Client, on behalf of themselves and on behalf of theirpersonal representatives, executors, heirs, beneficiaries, successors, and assigns (collectively“Releasing Parties”), hereby knowingly, intentionally, and voluntarily waive, release, discharge, indemnify, promise not to sue, and agree to hold harmless CAL, its owners, officers, directors shareholders, employees, and agents, including but not limited to the RNs (collectively “Released Parties”) from and against any and all losses, costs, claims, demands, causes of action, injury, damage, and liability whatsoever (including, but not limited to, court costs and attorneys’ fees), whether presently known or unknown, suffered by the Releasing Parties (“Claims”) as a result of or in connection with past, present, or future medical care, treatment, referrals, or diagnoses rendered by third party medical providers and clinicians.
